I plan on using 5 hours and 46 minutes of pto today. Upt will cover the rest of my 8 hour shift and tomorrow. Going on a small vacation to deal with this burnout. Do I have to report my absence if it’s only two days?

I went to a telehealth virtual doctor with mdlive and im trying to figure out who can i go to to get this signed? mdlive said they dont fill out health care provider forms and i dont wanna get fired

Just some advice. Never put your height, weight, date of birth, and marital status on your resume. I received one with those on it recently it’s not something I want.
