Medical Leave in Addition to Mat Leave?

Struggling to come to an agreement with a startup who is writing the mat leave policy for me. My current job offers 22 weeks, and this smaller start up has agreed to offer 12 weeks + medical leave.

I understand medical leave is a general term, so what would you assume by this? Would that be FMLA and unpaid?

I would also be leaving behind all other fertility benefits such as 3 cycles of IVF. Has anyone else seen IVF and similar benefits at a series B startup?
