Patient with AKI - improving.
Pre renal AKI - unknown heart function. Given >35ml/kg/hr fluid in a fast bolus
Sudden onset pulmonary oedema and driven into T1RF (PO2 6 on BG) But only requiring 2-4L of O2 (your consultant and NIC laughs and mocks saying trivial)
Fluid status ++++ positive balance, hypervolemic, jvp raised, oedematous slightly.
CXR shows enlarged heart but no history of HF.
But patient has AKI.
Doctors, what would you do?
